Log new pieces from here, with the names you already play with filled in from your own log. First, connect the Google Form that feeds your sheet — this site writes through your form, so it needs to know which one.

1. Open your form for editing, then ⋮ → Get pre-filled link.
2. Put anything at all in every field, then Get link → Copy link.
3. Paste it below. Nothing is submitted — only the field ids are read.

Your form's Composer question needs to be multiple-choice with Other switched on — this site submits every composer through Other, since it cannot read your option list.
